2. Wrong application form for downloading.
https://www.mzv.cz/beijing/zh/consular_information/x2015_05_15/index.html I downloaded application form in PDF again, I found it is still the wrong version. In 2016, when I apply for the Visa, the Chinese staff told me impatiently you filled the wrong form. However, today, PDF is still wrong. This application form, even in English website, is wrong.
Today, when I visit the Chinese website for consulate general of Czech Republic in Shanghai, I notice their information and application form are correct, detailed and clear.

Because the information on website is not clear, we called embassy and wrote email.
If we wrote email in Chinese to inquire, we cannot get reply. If we call to Chinese-line, they seldom answered the phone, and hung up phone after several sentences reply. When we called in English or wrote email in English, they reply very nicely.
In a brief conclusion(this word is only to embassy):
1. We cannot accept that the website is wrong and no one care to improve it at least in 1 and half year.
2. We cannot accept because of the unclear information, we inquire embassy, but Chinese staffs talk to us impatiently, and do not reply email.
3. We cannot accept the unclear information wasted our time and money.
